Authorization for Release of Health Information
Mee Memorial Health System maintains records of the services we provide you. We do not disclose your protected health information to others unless you give us permission or the law authorizes us to do so.
If you would like to request a copy of your medical record from Health Information Management, please download an Authorization for Release of Health Information. To contact the Health Information Management Department by phone 831-385-7183 or stop by Monday- Friday 8:00 am-4:30 pm.
